The Farm at Prophetstown, Trails for Tails, A 5K Run for The Farm Animals
Read this Acknowledgement of Rules, Risk and Waiver of Liability carefully and in its entirety
It is a binding legal document. If you are under the age of 18, this form must be signed by you as the participant AND by your parent or legal guardian. I know that participating in The Farm at Prophetstown 5K or Fun Run is a potentially hazardous activity and I should not enter and participate unless I am medically able and properly trained. I acknowledge and assume any and all risks associated with this event including, but not limited to, traffic on the course route, falls, contact with other participants, and the condition of the course, including, but not limited to, curbs, cars, uneven pavement, potholes, rocks, and objects on the course surface. Knowing and appreciating these risks and in consideration of your acceptance of my entry, I hereby for myself, my heirs, representatives or anyone else claiming on my behalf, covenant not to sue, and waive, release, and discharge The Farm at Prophetstown, its volunteers, and sponsors, Indiana DNR, and/or anyone else acting for or on behalf of The Farm at Prophetstown 5K or Fun Run from any and all claims of liability for death, personal injury, or damage of any kind arising out of my participation in this run. This Acknowledgement of Risk and Waiver of Liability extends to all claims of every kind whatsoever. I also consent to emergency treatment in the event of injury or illness. I grant full permission to The Farm at Prophetstown and/or any person or entity authorized by it to use my name, age, date of birth, finish place and finish time in the public domain. I further grant full permission for the Farm at Prophetstown to use any photographs, recordings, or any other record of this event for any purpose.

Example Items to Bring on Race Day
Common items to bring and use before the race include:
- A skin lubricant to limit blistering or chaffing.
- Sunscreen; ideally with 15 SPF or higher protection.
- A running watch or a GPS device you can easily carry. Race App not included in this run.
- Water
- An armband for your phone or a similar solution to carry your phone.
- Band-aids.
- Extra clothing such as a jacket.
- Snacks and Energy bars
- Small personal belongings such as an ID, cash or car keys. The Farm at Prophetstown waives all responsibility for lost or stolen items.
Additional Running Tips
A 5K race is a long race so expect a few challenges throughout the process. Take measures to prepare for injuries, accidents or even just congestion at specific points in the race.
The primary tip for any long race is to enjoy the process. Enjoy the journey and each step of the race. Reaching the finish line is an accomplishment so enjoy the race and the achievement of improving your personal time, reaching the finish line or even just getting out to the race and doing your best to complete the race.
Typical 5K race rules:
- Start: Participants must line up in the appropriate starting corral. Sign up begins at 10:00 am, April 18, 2026.
- Course Etiquette:
- Stay on the designated course.
- Run on the right side, pass on the left.
- Yield to slower runners.
- Water Stations: Approaching water stations carefully, avoiding sudden stops to not disrupt other runners. Water is available before, during, and after the race at designated areas.
- Race Time Limit: Race time expires after 75 minutes.
- Race Number Visibility: Participants must wear their race bib visibly throughout the race.
Important Considerations:
- Read the waiver carefully: Read the full waiver before signing to understand the terms and conditions.
- Consult a doctor if needed: If you have any medical concerns, consult your doctor before participating in a 5K race.
- Minors and waivers: If a participant is under the legal age, a parent or guardian must typically sign the waiver on their behalf.

In signing the Acknowledgement of Risk and Waiver Liability I hereby acknowledge and represent: (a) that I have read this document in its entirety, understand it, and sign it voluntarily; (b) that this Acknowledgement of Risk and Waiver of Liability is the entire agreement between the parties hereto and its terms are contractual and not a mere recital; (c) by selecting the ‘I agree’ button, I am signing this document electronically. I agree that my electronic signature is the legal equivalent of my manual/handwritten signature on this document
REQUIRED FOR ALL PARTICIPANTS UNDER 18 YEARS OF AGE: PARENT OR GUARDIAN’S AUTHORIZATION FOR MEDICAL CARE AND CONSENT AGREEMENT:
I certify that I am the parent or legal guardian of the above-named participant in the ACTIVITY. On behalf of myself and my spouse, partner, co-guardian or any other person who claims the participant as a dependent, I have read the above agreement, I understand the contents of this Acknowledgement of Risk and Waiver of Liability, assent to its terms and conditions, and sign this Acknowledgement of Risk and Waiver of Liability of my own free act. I acknowledge that my dependent and I have agreed to the terms and conditions of my dependent’s participation in the ACTIVITY, and I hereby give my consent to participation by my dependent in the ACTIVITY, and to receive medical treatment determined to be necessary. I further agree to hold harmless, indemnify and defend The Farm at Prophetstown from and against all claims, demands or suits that my dependent has or may have.
